error coin , planchet flaw , ??? interesting , mystery coin

As you said, maybe a planchet flaw or more probably a die defect. This could be one of the last coins struck, after the die had broken.

My first hunch would also be a die crack. But was this a struck coin ? Looking at the edges i wonder if this suggests a cast flan ?

The one thing that's obvious to me is that no one who would put a Kalima on a coin, or perhaps a temple token, would deliberately superimpose on it a strange image looking vaguely like a barbell, in such a way that the letters would look like its hind legs. Since I can't come up with any way someone would deliberately do such a thing, a minting mistake or flaw seems the only credible explanation.
