Looks like this one has been known since 2015 at least but I hadn't heard of it until now: https://www.coincuriosity.com/view/2009-ten-pence-struck-with-lion-reverse.html

Two or three examples known - maybe found in a set?

I posted about one on sale on ebay some time ago,
despite the seller being reputable (ecr64), the coin was met with (unwarranted imo) scepticism.

Although there are people who are able to alter coins to make them look like mule errors, and these altered pieces are indeed to some extent being offered for sale as genuine error coins, it is believed that this mule error indeed exists. As is normal, research and so also knowledge have advanced over the last 7 years. It is thought that it was part of one or more specific thematic sets that had very low sales figures. I wonder why the pieces that I have seen were loose coins though. Are so many sets being broken up?

One was sold by Dix Noonan and Webb auctioneers, it is indeed genuine and in the same vein as the 1983 mule 2p coin.

Oh interesting. To me it looks real enough - if anything I wonder about whether it was a deliberate fabrication.

Do you know if it sold in that ebay sale? The London Coins and Dix Noonan Webb were the only two sales I was aware of.

Mint sport I think quite likely for these - what were they doing striking 2009 10p with the pre-2008 reverse?

I've never seen any in any sets - so they would appear to be loose coins.
I will ask the seller what happened to this one and report back....

Yes hard to say. Apparently at least one was found in a set but all seems a bit unclear at the moment.

Now confirmed by the Royal Mint Museum. I wonder what happened to the "Swiss" specimen?
