Kavad I, recognisable by the two ribbons crossing the outer ring at the sides of the crown and the double margin on the reverse.

The mint is AYLAN, (previously identified as Eran Khwarra Shapur, laser Shush) now said to be at or near Eran-asan-kar-Kavad (Hulwan) in Media

Again I'm afraid I can't make out the date.
