DHP on 1937, 38 and 45 Dollars

Hi Guys!

I just got my shiny new 2020 Charlton catalog, so I'm fixing up my collection. and I ran across 'DHP' and 'QHP' listings in early dollars. This stands for doubled (or quadrupled) HP, designer's initials under George's bust. Can anybody tell me WHY just the initials were doubled and How (as re engraved, repunched, tooled, etc.)?

Found a 1947 THP :)

This was done in the mint, so there's no question of tooling. Can't think of a scenario that would cause this. It looks most like a mechanical error when the punch was applied to the die, as there was no attempt to hide or polish away the underlying punchmark. Bizarroïde.
