how did this mark came under the integration on this coin ?

Damaged die, I think. Someone dropped a tool or another die on the die that struck this coin.

does these die damage occur often on mints ,and also do these occur only in old coins(during 1994 etc) or in new coins also

Accidents happen in all times. I remember seeing similar damage on a Roman coin. For a very modern coin struck with a damaged die see here.
